All of the forms listed below can be downloaded online
by clicking here, or you can request
that an application packet be mailed to you by clicking here.
Application Form and Fee
Please complete and return the application form to
the Admissions Office. A nonrefundable $85.00 application fee must accompany
each childs application.
Teacher Evaluations
If your child is applying for pre-K or kindergarten,
please give the Teacher Recommendation for Pre-K/Kindergarten
Applicants to your childs current preschool teacher
and ask that the form be sent directly to the Admissions
Office when completed. Please note that applicants for
future years should not have the teacher recommendation
completed until the year prior to pre-K or kindergarten.
If your child is applying for grades 1-5, please
give the Lower School Teacher Evaluation to your childs current
primary teacher and ask that it be returned directly to the Admissions
Office in the envelope provided.
Applicants to grades 6-12 should provide current
year Math and English teachers with the appropriate teacher evaluation
forms and ask that they be mailed directly to the Admissions Office.
Parents Questionnaire
Complete the questionnaire carefully and thoroughly and return it to
the Admissions Office.
Student Questionnaire
Students applying for grades 9-12 must complete this questionnaire
and return it to the Admissions Office.
The Episcopal Code
Students applying for grades 9-12 should read and respond to
information about Episcopals Honor Code and Council and return
it to the Admissions Office.
School Transcript/Grade Report
Provide Episcopal with the most recent academic transcript
from your childs current school by completing the transcript release
for and sending it to the current school.
Testing
Grades PK-1: Development readiness
screening should be scheduled for one of the fall or winter test dates
in the year
prior to enrollment. Contact the Admissions Office for more information
on scheduling screening.
Grades 2-12: All applicants must take Episcopals admission
test. Please schedule your student for testing by contacting the Admissions
Office to schedule a testing date
Campus Visits/Tours
Arrange for a campus touring by contacting the Admissions
Office. This can be done prior to submitting an application or at any
time during the admissions process. We recommend visits for students applying
for grades 6-12.

Financial
Assistance
Episcopal offers financial assistance to admitted applicants
on a need basis. Families wishing to apply for financial assistance should
indicate their interest by checking the appropriate box on the admissions
application form. A financial aid packet will be sent after January 1.
